At Crystal Disability and Nursing Services, we understand that people with disability require assistance as they transition in various stages of life. These transition processes may be of educational, developmental, social, vocational, physical or emotional nature.
Our team provides support coordination for individuals and their families to plan and prepare for these transitions, including the identifying potential challenges or opportunities to success. Our supports is tailored to ensure the participant achieves independence and their goals.
